Please bring the following items with you:
- Completed New Patient Forms if you choose to print from our website
- Insurance cards - Medical and Dental
- Driver’s license
- A list of your current medications
- Names & phone numbers of your current physicians
Consultation
All services require a consultation which will determine if same day service is possible.
- We will ensure you are thoroughly informed about your treatment.
- Your comfort and well-being are the primary concern of our office.
Post-Operative Care
Dr Biederman and his staff strive for your smooth and uneventful recovery.
- We will thoroughly explain the post-operative instructions.
- We will call you for an update on your progress and give you an opportunity to ask additional questions.
